FLS Registry

The Fundamentals of Laparoscopic Surgery (FLS) program is a comprehensive, educational module and assessment tool designed to teach the fundamental knowledge, clinical judgment and technical skills required in the performance of basic laparoscopic surgery. The educational module consists of a web-based multimedia presentation of didactic content and watch & do exercises that focus on manual skills training. The FLS assessment includes two components, a computer-based cognitive assessment and a performance-based manual skills assessment. Individuals who successfully complete both the educational and assessment components of the FLS Program will have demonstrated the basic knowledge, judgment and technical skills fundamental to the performance of laparoscopic surgery, and are issued an FLS certificate.

This Registry lists individuals who have successfully completed the FLS program, their certificate issue date; and, if applicable, National Provider Identifier (NPI), a unique identification number for health care providers covered under the Health Insurance Portability and Accountability Act.

Effective October 1, 2012, FLS no longer issues certificates that are valid indefinitely, instead, all certificates must be renewed every 10 years by successful completion of the entire test (both cognitive and skills components).

• Any certificate issued October 1, 2012 or later is considered valid until no later than ten years from the original certificate issue date.
• Any certificate issued prior to October 1, 2012 is considered valid until no later than October 1, 2022, regardless of the original certificate issue date

Non-inclusion in the Registry may be for any of these reasons:

• Individual has not taken the FLS exam
• Individual has not achieved a successful score on the FLS exam
• Individual has taken the FLS exam but score results are pending
• Individual has achieved a successful score on the FLS exam but chooses to remain unlisted.
